Beta-hydroxybutyrate (BHB) is a ketone body associated periods of fasting, prolonged exercise, or ketogenic diets when glucose levels are low. Its chemical structure is that of a short-chain fatty acid with both hydroxyl (-OH) and carboxylic acid (-COOH) groups.

BHB exists in two enantiomeric forms: D-β-hydroxybutyrate and L-β-hydroxybutyrate. D-BHB is the biologically active form produced in the liver during ketogenesis & is used as an energy source.

Urine ketone test strips actually measure acetoacetate (another type of ketone body) and are not as accurate as blood tests for beta-hydroxybutyrate. Acetoacetate can fluctuate depending on hydration levels, time of day, and how adapted the body is to burning fat. As ketosis progresses, more of the body’s ketones may be converted into BHB to be used for energy, which might lead to lower acetoacetate levels in urine. This can give a false-negative result even though BHB levels in the blood are elevated. Results can be affected by factors such as hydration (diluted urine can lead to lower readings) or how long the urine has been in the bladder.
